As part of the registration process, you and your parent/guardian are required to read and review the following documents:
Technology Resource Access Agreement for Affiliated Programs (links to document in English, Spanish):
Information for Parents/Guardians Regarding Mines Technology Resource Access (English, Spanish):
You and your parent/guardian may then complete and sign the Minor’s Technology Resource Use Waiver Form (English, Spanish):
Your parent/guardian are also required to complete and sign the Colorado School of Mines Release and Liability Waiver (English, Spanish):
Pdf scanning details: Consider adding your signature to the pdf documents electronically using Adobe Acrobat or other pdf reader that allows adding signatures. Alternatively, you may print the document, complete and sign using a pen, and then scan as a pdf using a mobile phone pdf app (options here) or take a well-resolved image using a mobile phone camera.
At end of form, you will be required to:
- Upload signed Minor’s Technology Resource Use Waiver Form (*required)
- Upload signed Colorado School of Mines Release and Liability Waiver (*required)
